Our services

Our specialist team provide care for adults and children with eye conditions. We visit five rural locations: Bourke Hospital, Brewarrina Hospital, Cobar Hospital, Lightning Ridge Hospital, Walgett Hospital.

Our services include:

  • Paediatric ophthalmology outpatient clinics
  • Cataract Surgery
  • Oculoplastics Surgery
  • OCT, Retinal diagnostic testing
  • Visual Field Assessments.

Inclusion criteria

All general ophthalmology and paediatric opthalmology referrals. 

Exclusion criteria

Emergency referrals. 

Referral Form

If your patient is eligible for treatment, please fax or email this referral form to Fax: 02 9382 2690 or email: Elyssa.brennan@health.nsw.gov.au.

Please ensure you have completed the mandatory information on your form:

  • Patient name
  • DOB
  • Postal address
  • Contact phone number
  • Clinical reason for referral/priority
  • Visual acuity, each eye
  • If possible – recent optometrist eye exam report

Once we have received a completed referral form, a member of our team will contact your patient via letter with appointment details.

Usual wait times may vary. An urgent referral will be seen within 4 weeks. A non-urgent referral may be seen in up to 12 months.

If your patient needs to change or cancel their appointment they should telephone 0418 322 705 or 0419 111 687.

We are a Medicare service. You must provide a referral that is addressed to one of our specialist team. Your patient will be bulk-billed.
