COVID Community Care

Care of COVID positive patients isolating at home 

If you have COVID and are isolating in the South Eastern Sydney Local Health District region we provide a home monitoring service delivered by a team of specialist nurses and doctors. This service is free for all people (citizens and non-citizens) with COVID-19.

Our team is notified of all COVID positive results in the region, we will contact you as soon as we can to provide you with information and allocate you to one of our specialist teams to monitor your health whilst you isolate at home. If you live in South Eastern Sydney and have not heard from us within 3 days of receiving your COVID result please contact the COVID Community Management Centre on 0499 435 968. 

Our service is staffed from 8am to 8pm, if you are feeling unwell after hours our preference is that you use an ambulance to come to hospital.

If you need urgent medical attention at any time call 000 and ask for an ambulance. Tell them that you have COVID-19 and need to go to the hospital.
