Acute Care Team (ACT)

What we do?

The Community Mental Health Service conducts urgent community-based assessment and short-term treatment interventions for people with a mental illness in crisis. We also provide intensive support for people with prolonged and severe mental illness and associated high-level disability, non-urgent, and continuing-care services for people with a mental illness and their families or carers in the community.

What to expect?

Our service uses an assertive outreach approach that may result in a clinician being involved with care for extended periods of time or providing more episodic care. A clinician can help with managing mental health declines, cope with distress and ways to improve mental health and wellbeing.
