SES HealthPathways Program (South Eastern Sydney)

SES HealthPathways

HealthPathways is an online tool aimed to support GPs during patient consults with information on assessment, management and guidance regarding when and where to make referrals. It also has many positive benefits for the hospital system and patients.

SES HealthPathways is a partnership between South Eastern Sydney Local Health District (SESLHD), Central and Eastern Sydney Primary Health Network (CESPHN) and St Vincent's Hospital Network Sydney. The program also works closely with Sydney Children's Hospital Network. SESLHD manages this program on behalf of the partners.

There are currently over 580 pathways, with more in development. The pathways are regularly reviewed to ensure content remains up to date. We have approximately 90,000 page views each year.

Access is only available to healthcare workers, generic log in details are available by clicking the 'Register now' button on the SES HealthPathways log-in page (applications vetted by the HealthPathways team) and can be shared between colleagues.
